Dr. Svetlana Kotlin, DPT
In my practice I combine manual therapy, modalities and exercises which ever benefits my patients the best at a given moment. Physical therapy session is a dynamic process and should be tailored to patient's specific condition at a specific moment of their recovery. My treatment is successful due to accuracy of diagnosis of the underlying problem(s) and application of the appropriate treatment technique. For years, I have been challenged by treating complex cases. Physical therapy usually begins with correcting any mechanical or leg-length asymmetries, utilizing muscle energy therapy, manual and neuromuscular techniques (manual therapy). Modalities (deep oscillation therapy, ultrasound, LASER, functional electrical neuro-muscular re-education of the lumbar dorsal spine musculature, decompression therapy and more) are used as preparatory tools to "get body ready for healing". Stretching excessively tight lumbar and pelvic muscles, and strengthening weak and inhibited muscles addressed via exercises to maintain (achieved through therapy session) structural balance. Most exercises performed on inclined vibrational board Prohabilitator. Core stabilization exercises usually begin in the neutral spine position or a pelvic position to minimize acute pain. Patients are progressed through core stabilization exercises at increased intensity and angle of Prohabilitator and challenging dynamic spinal stabilization, functional positioning as well as balance and proprioceptive activities. If this seems rational to you please call to schedule an appointment for consultation.

Dr. Kotlin says...

I treat lower back pain with LASER, Decompression, Taping, Muscle Energy technique, and more. The low-back syndrome, and in particular sacroiliac slippage (subluxation, dislocation, nutation), is common in those doing heavy labor and is also one of the most common and often ignored conditions that can beset all of us. The sacroiliac problem usually arise from twisting or turning motions. When the body is in this position, the sacroiliac joint is opened slightly, one set of muscles pulling on the ilium, while another pulls on the sacrum and the slide of joint results in impingement on a nerve with instantaneous, pain: a dull aching sensation in the low back, on the side of the slippage. It may occur as a pain radiating down the back of the leg (sciatic nerve) or around the side and to the front of the leg (anterior cural nerve). The patient may hear a click when the joint slides, or there may be no noise sensation. Numbness or mild tingling down the leg may occur along the nerve pathways already described. At times the condition may not be noticed until the patient attempts to rise from a sitting position, or turn in bed only to find that his back is stiff and must be "loosened" before he can move. The ramifications of this difficult condition: enigmatic headaches and neck difficulties, tension in the upper shoulders, stomach upsets, hernia-like pains in the groin, shooting pains down the leg, knee difficulties, and pains in the feet and ankles.  Usually only the symptom area was treated, and thus only partial success was achieved. I have developed methods of treating this common and agonizing problem with speed, yet at the same time keeping the patient ambulatory and productive.
