As someone who has been evaluation and treating the most complex and chronic musculoskeletal pain problems for some time, including post-surgical back and neck pain, the most important consideration is the time and thoroughness of the history and the examination. This helps to create a highly specialized patient centered approach to better diagnosing and treating the the individual patient. More often than not, the initial face to face consult with the patient last 3 - 4 hours face to face.
