He wrote a drs note and mentioned in it that he physically examined me, but never did so how can he know how I’m feeling? He also raised his voice and hollered at me when I asked him that day how he can think and say what he did not having asked me any questions about how I’m feeling since last visit or coming off of medicine for symptoms to see if it was the drugs or the condition or even doing a physical exam like was done the first time He also mentioned concussion in office but not in note
