This content was provided by Sumir Sahgal "Dr. Sumir Sahgal, a Board-Certified Internist, is the Medical Director of Essen Medical Associates, P.C., a comprehensive multi-specialty medical group with 6 offices in the Bronx, N.Y, and EssenMED House Calls, a physician-based house call service caring for chronically-ill homebound patients living in New York City and Westchester. Under Dr. Sahgal's leadership, Essen Medical Associates was one the first to achieve the 'Level 3 Patient Centered Medical Home' designation in the state of New York State. Additionally, as one of the 'Early Adopters' of Electronic Health Records in New York City, Dr. Sahgal was recognized with a visit from Mayor Michael R. Bloomberg and the Health Commissioners of both the city and state in 2008. Most recently, he was inducted into the inaugural class of Health Information Technology (HIT) Fellows awarded by the Office of the National Coordinator of Health Information Technology for the U.S. Department of Health and Human Services (HHS) and was honored with the 'New York Health IT Practice Champion Award' by the New York eHealth Collaborative in October 2012. Leveraging the use of HIT by arming providers with electronic health records, in 2005 Dr. Sahgal launched EssenMED House Calls to extend primary care services beyond office walls and into patient homes. EssenMed House Calls is now the largest private practice-based medical house call program in the New York State. A critical component to reducing avoidable hospitalization and visits to emergency rooms, EssenMED House Calls also partners with a number of local area hospitals to provide Transition of Care services, interim care to patients following hospital discharge A graduate of Maulana Azad Medical College, New Delhi, India, Dr. Sahgal completed his Internal Medicine Residency training program at Brookdale University Hospital and Medical Center, Brooklyn, NY, and holds a Master's degree in Health Policy and Management from New York University. Among his notable accomplishments and leadership roles, Dr. Sahgal is currently the Chief Medical Office of Balance Accountable Care Organization. He also serves on a number of committees including, the International Medical Graduates Committee of the American Medical Association (AMA) and is the Chair of the Committee for International Medical Graduates of the Medical Society of the State of New York (MSSNY). He is also an active member of the American College of Physicians, the Bronx County Medical Society, the American Medical Directors Association, and the American Academy of Home Care Physicians."
