Nature of Complaint:
The physician had previously entered into a consent order of voluntary surrender of licensure on 01-02-2007 after the board received information from the PAP-NJ that the physician had admitted relapsing into the abuse of controlled dangerous substances.
Action Taken-Consent Order for Licensure:
The physician must prior to reinstatement successfully complete all application requirements and undergo a criminal history background check. The physician shall comply with the PAP monitoring program subject to the following:
The physician shall abstain from all psychoactive substances including alcohol unless prescribed for a documented medical condition by a treating health care professional.
The physician shall participate in individual psychotherapy.
The physician shall attend a minimum of three Alcoholics Anonymous meetings per week.
The physician shall submit to random witnessed urine monitoring under PAP supervision.
The physician shall submit to continued monitoring by the PAP and meet on a face-to-face basis at least once a month for the first three months of the physician's return to the practice of medicine.
The physician shall ensure that the PAP submits reports regarding the physician's progress with the monitoring program every ninety days.

The Physician has a history of substance abuse and recently suffered a relapse.
The Physician self-reported his morphine abuse in December 2004. He enrolled in the Professional Assistance Program and enrolled in a treatment program for thirty days.
The Physician suffered a relapse in November 2006.
The Board has granted the Physician's request to Voluntarily Surrender his license to practice medicine in the State of New Jersey.
