The physician Voluntarily Surrendered his license to practice medicine in the State of Connecticut in settlement of the allegations contained in the above-reference case.
March 15, 2011-Reinstatement Consent Order
The physician has applied to have his license reinstated.
The physician's license to practice as a physician and surgeon shall be reinstated when the physician satisifes the requirements for reinstatement of his license. Immediately upon issuance, the physician's license shall be placed on Probation for a period of five years subject to various terms and conditions including but not limited to the following:
The physician shall participate in regularly scheduled therapy with a licensed psychiatrist or psychologist pre-approved by the Department.
The physician shall abstain from the ingestion of alcohol in any form and the ingestion, inhalation, injection or other use of any controlled substance and/or legend drug unless prescribed or recommended for a legitimate therapeutic purpose by a licensed health care professional authorized to prescribe medications.
The physician during the first and second years of the probationary period shall submit to twice weekly random urine screenings.
The physician during the third, fourth, and fifth years of probation shall submit to screens on a weekly basis.
The physician shall attend support group meetings on an average of eight-ten times per month and shall provide quarterly reports to the Department.
The physician during the period of probation shall only practice medicine in an office and practice setting that includes other licensed physicians.

The Maine Board of Licensure in Medicine staff on 10-08-2009 received written confirmation from the Maine Medical Professional Health Program that the physician had tested positive a second time for ethyl glucuronide, a metabolite of alcohol.
The physician admitted that he violated the 07-09-2009 Consent Agreement for Conditional Licensure on more than one occasion by testing positive for the presence of a metabolite of alcohol and using alcohol.
