      • Nature of Complaint: Patient seen by PCP (Internal Medicine) for aspiration of knee several times. When culture returned positive, patient was referred to Insured (ORS). Patient first seen by Insured 5/6/2008 for consult and then underwent surgery for wash-out of knee on 5/9/2008. Patient was seen in follow-up a week later, then seen by PA two more times. When knee was not improving, patient was referred to partner, but patient followed up with a different doctor instead. Insured is aware patient's knee swelled up again in October 2008.
