Mintz, D.O.; License # OS003583L; NATURE OF COMPLAINT: The osteopathic physician is unable to practice osteopathic medicine and surgery with reasonable skill and safety to patients due to impairment. ACTION TAKEN: The Pennsylvania Board of Osteopathic Medicine indefinitely suspended the osteopathic physician's license for no less than three years. The Board immediately stayed in favor of no less than three years of probation subject to terms and conditions.
