Dr. Martin abruptly discharged my 81 year old father from the hospital without any case management for discharge planning, physical therapy evaluation or medication change reconciliation. After 10 days of intravenous and oral blood pressure medications to stabilize a new diagnosis of high blood pressure, he was going to send him home without any medication. My father was only walking 10 feet to the bathroom with assistance. He suffered 2 days of a never before incontinence after discharge.
