Dr. Gallagher's Care Philosophy

The first principle of care for patients with epilepsy is to strive to achieve seizure freedom with no side effects and enable them to engage in all activities with no restrictions. This goal can often be achieved with optimal medical and, if appropriate, surgical therapy. The second principle is to understand that patients with epilepsy often experience other neurological issues in addition to seizures which require treatment to maximize their quality of life. Therefore, for patients with epilepsy, I strive to assist them with these other conditions including headaches/migraines, depression, anxiety, attention deficit hyperactivity disorder (ADHD usually involves a referral to a mental health provider), sleep disorders (often a referral to our sleep center), and cognitive/memory issues (often involves optimization of their seizure medications, screening by one of our neuropsychologists, and, if appropriate, referral to Vanderbilt's comprehensive rehabilitation center).

    I am ABPN board certified in neurology, clinical neurophysiology and epilepsy and I specialize in the evaluation and management of patients with epilepsy (recurrent seizures). Because many other conditions can mimic seizures (e.g. syncope, psychogenic nonepileptic events, migraine, stroke, sleep disorders, etc), the first step is to determine whether or not the patient is likely to have seizures.  This may involve continuous EEG and video recording until the patient has a typical event. Usually this monitoring is done for 3-7 days in the hospital, but outpatient options are also being developed.  If the diagnosis of epilepsy is made, medications are first-line treatment with the goal of stopping seizures with minimal, if any, side effects.  The possibility of surgical therapy will be discussed with patients who still have seizures despite adequate medical therapy; oftentimes, this can be very effective.  Dietary and experimental therapies are other options.
