    Nature of Complaint:

    The physician on 02-02-2010 petitioned the Vermont Board of Medical Practice to find he has satisfied the conditions of the 03-02-2005 Stipulation and Consent Order, as amended in 2009, and to fully reinstate Vermont medical licensure. The physician asserted that he has fully complied with all of the conditions of his 2005 Stipulation with the State of Vermont. The Medical Director of the Vermont Practitioner Health Program submitted a letter dated 02-09-2010 which stated that VPHP is in full support of the release of all conditions on the physician's medical licensure.

    Action Taken-Notice of Entry of Order

    The Vermont Board of Medical Practice approved the physician's request for relief of conditions on the physician's license by the 03-02-2005 Stipulation and Consent Order. The physician has a full and unconditional license .
