Dr. Forman was born in Quantico, Virginia and raised in Fort Lauderdale, Florida. A graduate of the University of Florida, he completed his medical training at the University of Miami serving as Chief Resident in the department of Anesthesiology Perioperative Medicine and Pain Management. He is Board Certified and a diplomate of the American Board of Anesthesiology, the American Board of Pain Medicine, and is a Fellow of Interventional Pain Practice.
Dr. Forman believes in spending time with his patients educating and informing them to help manage their condition. He enjoys the challenges associated with managing both simple and complicated pain syndromes utilizing advanced interventional therapies. He is a member of multiple professional Associations including the American Academy of Pain Medicine, Texas Medical Association, American Society of Interventional Pain Physicians, and World Institute of Pain among others.
Dr. Forman has been in private practice in South Texas for over 20 years previously serving as chief of anesthesia services at Valley Baptist Medical Center-Harlingen. Dr. Forman currently holds multiple local hospital staff privileges including Valley Baptist Medical Center Harlingen and Brownsville.
Dr. Forman enjoys spending his time outdoors and with his children. He is married and the father of three.
