    • Nature of Complaint: OUT OF STATE CLAIM: MO. Reported by the North Carolina Medical Board INCIDENT LOCATION: HANNIBAL, MO /// PRACTITIONER COMMENTS: The patient was an elderly male on dialysis. I was successful in creating a left arm AV fistula in this patient and followed him regularly for management/maintenance of his fistula. Patient presented to the ER with bleeding from the fistula. I was able to perform a full thickness suture repair of the bleeding site in the ER with good result. Later, I saw him in follow-up. I noted some dehiscence of the skin around the repair site but there was no bleeding and the fistula was functioning well. In order to reduce the risk of future bleeding due to the noted dehiscence, revision of the fistula was recommended. he was scheduled for the procedure the very next week, but unfortunately he passed away the day before the scheduled procedure, apparently due to bleeding from the fistula.
