    1. The physician shall abstain from alcohol, all controlled dangerous substances and potentially addictive substances unless prescribed by a treating health care practitioner.
    2. The physician shall participate in at least three support group meetings per week.
    3. The physician shall undergo random, witnessed urine monitoring once per week for a minimum period of one year.
    4. The physician shall ensure that the PAP submits reports every ninety days to the board regarding the physician' progress.
    5. The physician shall attend face-to-face meeetings with a staff member of the PAP on a monthly basis.
