    Action Taken:

    The Medical Board of California has Revoked the physician’s license to practice medicine in the State of California. The revocation is Stayed and the physician is placed on Probation for a period of two years, subject, but not limited to, the following terms and conditions:

    1. The physician shall successfully complete a medical records keeping course during the first six months of probation.
    2. The physician shall enroll in an ethics course, to be completed during the first year of probation.
    3. The physician shall successfully complete the Wrong-Site Surgery Course offered by the PACE program no later than six months after enrollment.
    4. The physician shall obey all laws and all rules governing the practice of medicine in the State of California.
    5. The physician shall submit quarterly declarations, stating whether there has been compliance with all of the conditions of probation.
    6. The physician shall comply with the Board’s probation unit.
    7. The physician shall be available in person for interviews with the Board.
    8. The physician shall pay all probation monitoring costs, each year of probation.
