Nature of Complaint:
The Physician has violated the terms of his probation by consuming alcohol.
The Physician had relapsed and consumed alcohol and treated a patient while he was on call on November 29, 2004.
Due to his repeated violations, the Board has chosen to Revoke the Physician's license to practice medicine in Maryland at this time.

Physician tested positive for cocaine on chemical drug screen obtained 12-19-00. This is a violation of the reinstatement order issued 11-24-99. The order stated if the Physician tested positive his license would be revoked.
October 23, 2002-Reinstatement of License
Physician petitioned for reinstatement of his license in April 2002. The Board granted the reinstatement with Probation for 5 years with the following terms:
Physician shall remain under Advocacy Contract for 5 years and be subject to random urine screens.
Physician shall completely abstain from alcohol and the use of CDS unless the Physician is a bona fide patient of a physician who is aware of this contract.
Physician wiil remain in psychotherapy at a frequency recommended by the therapist.
Physician will notify Board his propsed practice settings and that they be supervised.
Physician will inform all employers of the Advocacy Contract.

Physician submitted letter to Board surrendering his Maryland license due to his inability to work due to illness. Physician produced positive drug screen on 12/29/98 which was a violation of his Advocacy contract.
September 24, 1999 - Reinstatement of License
The Board recommended a reinstatement of Physician's license with a Probation of 5 years with the following conditions:
Board will approve of practice setting or any employment position submitted by Physician
Physician will inform his employers of the Advocacy Contract
If Physician cannot obtain employment in a group setting and resumes in a private practice, the Physician shall be monitored by a Board approved supervisor.
This relationship shall continue for a minimum of 1 year.
The supervisor shall inform Board of they determine Physician's practice constitutes a threat to the health and safety of it's patients.
If the Physician practices in a group setting,the employer must provide the same supervisory options as listed above.
Physician shall remain in therapy on a weekly basis.
Physcian shall remain under contract with physicians program for 5 years and remain compliant with the terms of the Advocacy Contract.
Physician shall be subject to immediate revocation of license upon notice of positive drug screen or other relapse.
