The patient's mother and her husband on more than one occasion, by telephone and in person requested the three-year-old child's medical records from the physician. The child's medical records from the physician as of 08-19-2009 have not been received. The physician failed to furnish in a timely manner without delays for legal reviews, copies of all reports and records relating to such examination or treatment including x-rays and insurance information.

- The Florida Board of Medicine has suspended the physician's license to practice medicine in the State of Florida until the physician personally appears before the Board to address the issues set forth in this case.
- The physician shall complete the Laws & Rules Course sponsored by the Florida Medical Association within one year.
- The physician shall pay a fine in the amount of $5,000.00 within thirty days.
- The physician shall pay costs in the amount of $5,421.97

The physician on two occasions failed to show up as agreed and provide a patient's medical records to the patient's mother. A Department Investigator on 03-20-2009 hand delivered a copy of a lawfully issued subpoena duces tecum to the physician ordering the physician to provide the patient's medical records by 04-02-2009. The physician failed to furnish in a timely manner without delays for legal review copies of all reports and records relating to such examination of treatment including x-rays and insurance information. The physician failed to comply with a lawfully issued subpoena of the Department.

- The Florida Board of Medicine has suspended the physician's license to practice medicine in the State of Florida until the physician personally appears before the Board to address the issues set forth in this case.
- The physician shall pay a fine in the amount of $5,000.00 within thirty days.
