Nature of Complaint:
The physician discontinued the practice of medicine in 1996 to pursue a business venture. The physician wishes to return to the practice of medicine and has filed an application with the board for reinstatement of his medical license.
Action Taken:
The physician has entered into a Stipulation and Agreement with the board for reinstatement of the physician's license to practice medicine in the State of Vermont subject to various terms, conditions and requirements to include a period of clinical supervision, monitoring, skills and knowledge and general re-training.
July 2, 2008-Amendment to Stipulation and Agreement
The board has approved Dr. Candace Fraser to act as an additional secondary supervisor for Dr. Giroux. All other terms of the 04-02-2008 Stipulation and Agreement remain unchanged.
April 1, 2009-Notice of Entry of Order
The Vermont Board of Medical Practice has approved the physician's request for relief of conditions on the physician's license by the 04-02-2008 Stipulation & Agreement. The physician has a full and unconditional license .
