Nature of Complaint
The physician has a history of alcohol abuse. On or about August 24, 2010 the physician notified the Board that he had relapsed on alcohol for the third time. The physician stated that he entered a Board-approved treatment facility for detoxification and subsequently entered another Board-approved treatment provider for treatment, which is not yet completed.
The Board has determined that there is clear and convincing evidence that the physician has violated the May 2006 Step II Consent Agreement and that his continued practice presents a danger of immediate and serious harm to the public.
The State Medical Board of Ohio has ordered that the physician’s certificate to practice medicine and surgery in the State of Ohio shall be immediately Summarily Suspended, as a pre-hearing action.
March 9, 2011 – Step I Consent Agreement
In lieu of formal proceedings, the physician entered into a Step I Consent Agreement with the State Medical Board of Ohio in which the physician agrees to various terms, conditions and limitations including and not limited to the following.
- The physician’s certificate to practice medicine and surgery in the State of Ohio shall terminate from the Summary Suspension effective September 8, 2010. Further the physician’s medical certificate shall be Indefinitely Suspended for not less than three years.
- Conditions for reinstatement have been established.
- Upon reinstatement, the physician shall be subject to various Probationary terms, conditions and limitations.
- The physician shall be subject to Required Reporting.
In the event the physician violates any term or condition of the Agreement, the Board reserves the right to institute formal disciplinary proceedings.
