Nature of Complaint:
The physician has been the subject of disciplinary action in another state.
The Florida Board of Medicine approved a Settlement Agreement entered into by the physician and the Florida Department of Health. The physician was issued a letter of concern, required to pay an administrative fine and costs, perform fifty hours of community service, complete five hours of continuing medical education in risk management, and present a one hour lecture/seminar on wrong site surgeries. The action was based on findings that on 11-14-2006 the physician performed a wrong-site procedure by injecting a left-sided transforaminal epidural instead of injecting a right-sided transforaminal epidural.
The physician has entered into a Consent Order with the Virginia Board of Medicine as follows:
- The physician shall comply with the terms and conditions of the Florida Settlement Agreement.
- The physician shall provide written notification to the Virginia Board of Medicine of any change in the Florida Settlement Agreement within ten days of such change.
- The physician shall authorize free communication between the Virginia Board of Medicine and the Florida Board of Medicine.

The physician has complied with the terms of the 06-15-2010 Consent Order. The physician currently has a full and unrestricted license in the State of Virginia.
