Residency training takes place in accredited hospitals or other healthcare facilities under the supervision of experienced doctors. Residency training lasts from three to seven years, and the exact duration varies from specialty to specialty. Residency is required for specialty board certification.

The Kentucky Board of Medical Licensure has issued an Order Terminating Second Amended Agreed Order.
Nature of Complaint:
The physician's prescribing practices were inappropriate. The physician failed to discontinue treatment when his patients demonstrated signs of abuse, misuse and diversion of controlled substances. The physician failed to meet the the minimum standard of care in the treatment of patients and documentation of treatment.
Action Taken:
Inquiry Panel B has ordered an Emergency Order of Restriction on the physician's license to practice medicine in the State of Kentucky prohibiting the physician from prescribing, dispensing or professionally utilizing controlled substances until further order of the board.
August 21, 2008-Agreed Order of Indefinite Restriction
The physician has successfully completed the “Prescribing Controlled Drugs” course at the Center for Professional Health at Vanderbilt University Medical Center. The physician has also enrolled in the CPEP chart documentation course. The physician has also implemented a number of changes in his practice including requesting KASPER reports on all new patients and periodically as indicated thereafter and requiring each new patient to sign a controlled substances contract.
The physician has entered into an Agreed Order of Indefinite Restriction subject to the following restrictions/limitations:
- The physician shall not prescribe, dispense or professionally utilize controlled substances until approved to do so by the board.
- The physician shall successfully complete the CPEP post-documentation course.
- The physician shall pay investigation costs in the amount of $2212.50 within six months.
December 5, 2008-Amended Agreed Order
The physician on 09-12-2008 attended the CPEP documentation seminar and has enrolled in the CPEP post-documentation program.
The physician has entered into an Amended Agreed Order with the board based upon the physician's request to resume the prescribing of controlled substances subject to the following restrictions/limitations :
- The physician shall successfully complete the CPEP post-documentation course .
- The physician shall maintain a controlled substances log for all controlled substances prescribed available for review by the board upon request.
- The physician shall be subject to at least one consultant review .
- The physician shall reimburse the board for the costs of each consultant review performed.
April 14, 2009-Second Amended Agreed Order
The physician has entered into a Second Amended Agreed Order with the board subject to the following terms and conditions:
- The physician shall successfully complete the CPEP post-documentation course .
- The physician shall maintain a controlled substances log for all controlled substances prescribed and permit the board's agents to inspect and obtain upon request for review by the board.
- The physician shall reimburse the board for the costs of each consultant review performed.
- The physician shall be subject to at least one consultant review .
- The physician shall pay investigation costs in the amount of $1050.00 within one year.
