The physician failed to comply with the terms and conditions of his board order dated 06-16-2005. The physician failed to submit documentation of the completion of the FMA Laws and Rules Course and fifty hours of community service.
Action Taken:
The board has issued a final order and has imposed the following obligations:
- The physician shall pay a Fine in the amount of $10,000.00 to the board plus costs.
- The physician shall complete one hundred hours of community service .
- The physician shall be subject to an audit for the next three biennia to determine compliance with continuing medical education requirements.
- The physician has been issued a Reprimand by the board.
