Nature of Complaint
In or about December 2007 the physician informed the Board that her hospital privileges at three hospitals had been subject to suspension. The physician was found guilty of operating a motor vehicle under the influence of alcohol and in May of 2008 the physician violated probation, which resulted in the Wayne County Municipal Court issuing a warrant. The physician has had subsequent interactions with law enforcement officials and city officials.
Following the evaluation at a Board-approved treatment provider the Board received notification that the physician was determined as impaired in the ability to practice according to acceptable and prevailing standards of care and required residential treatment for chemical abuse/dependence. The Board has not received confirmation that the physician entered the required treatment. Further the physician falsely answered a question on her license renewal application.
The State Medical Board of Ohio has ordered that the physician’s certificate to practice medicine and surgery in the State of Ohio shall be Summarily Suspended.
09/08/2010 – Board Order
The State Medical Board of Ohio has determined that the physician is impaired in ability to practice due to diagnosis of Cocaine Abuse and the physician has not entered treatment at a Board-approved provider. Further the physician made false, fraudulent, deceptive or misleading statements in relation to the practice of medicine. The Board has ordered the physician license to practice medicine and surgery in the State of Ohio shall be Revoked. The Order is effective 09/10/2010.
