Allegation of Complaint:

Nature of Complaint:
The physician on numerous occasions from approximately September 2009 through July 2010 diverted for his personal and unauthorized use sufentanil injection solution, Dilaudid injection solution, and morphine injection solution from a hospital and/or surgery center at which the physician worked as an anesthesiologist.
The physician on 07-09-2010 and on numerous occasions between September 2009 and July 2010 falsified medical records to indicate that medications the physician had diverted for his personal and unauthorized use had been discarded as waste.
The physician admitted to the Virginia Board of Medicine investigator that on 07-09-2010 the physician diverted sufentanil injection solution and self-administered the medication while on duty and treating multiple patients as a hospital anesthesiologist.
The physician's anesthesia records for six patients and operative and/or surgical records for four patients that the physician wrote on 07-09-2010 were illegible.
The physician on 07-14-2010 signed a Participation Contract with the Health Practitioners Monitoring Program. The physician on 10-12-2010 signed a Recovery Monitoring Contract with the Health Practitioners Monitoring Program which the physician shall complete on 07-16-2015. The physician's HPMP case manager reported that the physician has been fully compliant with his RMC contract and has been approved by HPMP to return to practice and shall be required to have a work site monitor.
The physician has entered into a Consent Order with the Virginia Board of Medicine subject to the following:
- The physician has been issued a Reprimand by the Board.
- The physician shall remain in HPMP and fully comply with the terms of his Recovery Monitoring Contract until the physician successfully completes the Program.
