Nature of Complaint:
The physician from 2004 to 2008 with regard to his care and treatment of four patients failed to document that he established a treatment plan with specific objectives for the management of the patients chronic pain and did not consistently document the reason for the selection of the medication or the administration schedule. The physician also failed to periodically review and document that he independently assessed the patients progress with the treatment plan. The physician also repeatedly authorized renewals of medications with additional refills without any medical indication other than the information provided to the physician by the patients or without first examining them. The physician also failed to appropriately monitor the patients use of controlled substances. The physician also failed to document the medical records with information indicating that the physician addressed the signs and symptoms of escalation and/or abuse of narcotic therapies despite having information to that effect.
The fourth patient presented to the physician only twice between 01-01-2006 and 01-11-2008, however, the physician wrote prescriptions for controlled substances on a continuous basis during this time. The physician prescribed 1320 tablets of hydrocodone and 660 tablets of diazepam during this period.
The physician currently is not treating chronic pain patients and refers out to pain management specialists in his area.
The physician has been issued a Reprimand by the Virginia Board of Medicine and the physician's license shall be subject to the following terms and conditions:
- The physician within nine months shall complete twenty hours of board approved continuing medical education in pain management.
- The physician shall complete fifteen hours of board approved continuing medical education in medical recordkeeping and documentation within nine months.
May 13, 2010-Terms Terminated
The physician has complied with the terms of the 07-15-2009 Order. The Virginia Board of Medicine has terminated the terms placed on the physician's license. The physician currently has a full and unrestricted license in the State of Virginia.
