Nature of Complaint:

The physician has been the subject of disciplinary action in another state.

The Rhode Island Board of Medical Licensure and Discipline on 08-13-2008 accepted the voluntary surrender of the physician's license to practice medicine in the State of Rhode Island subject to conditions for reinstatement. The action was based upon the physician writing and prescribing fraudulent prescriptions for fentanyl patches using two separate pharmacies and two different names and taking narcotic medications from patients for his personal use.

Action Taken:

The New York State Board for Professional Medical Conduct has Indefinitely Suspended the physician's license to practice medicine in the State of New York for a period of no less than twelve months and subject to various conditions.

Nature of Complaint:

The physician admitted to Rhode Island Task Force agents writing fraudulent prescriptions for fentanyl patches using two separate pharmacies and two different names. The physician also admitted to taking narcotic medications from certain patients for personal use. The physician was also charged by the Woonsocket Police with two felony counts of possession of controlled substances.

Action Taken:

The physician has entered into a Consent Order with the board subject to the following:

  1. The physician agrees to Surrender his license to practice medicine in the State of Rhode Island.
  2. The physician agrees to enter a residential treatment facility for evaluation and treatment of narcotic prescriptions.
  3. The physician agrees to enter into a five year monitoring agreement with the board and the Physician Health Committee of the Rhode Island Medical Society.

January 14, 2009-License Reinstated With Probation

The physician subsequently underwent intensive residential treatment in Williamsburg, Virginia and has signed and complied with a five-year substance abuse monitoring contract with the Physicians Health Committee.

The Rhode Island Board of Medical Licensure and Discipline has Reinstated the physician's license to practice medicine in the State of Rhode Island on Probation with a restriction that prohibits any prescription of controlled substances and subject to the following terms and conditions:

  1. The physician shall comply with his treatment and monitoring contract with the Physicians Health Committee.
  2. The physician shall not prescribe any controlled substances or apply for a DEA registration without prior board approval.
  3. The physician shall limit his clinical hours to those recommended by the Physicians Health Committee.
  4. The physician shall perform fifty hours of community service for each year of probation.

June 9, 2010-Reinstatement of Controlled Substances Registration Order

The physician petitioned the Rhode Island Board of Medical Licensure and Discipline for release from a restriction that denied the physician the right to hold a controlled substances registration.

The Rhode Island Board of Medical Licensure and Discipline has ordered that all restrictions concerning the physician's controlled substances registration are lifted .
