    Nature of Complaint

    The physician has been admitted to a Board-approved treatment provider for a minimum of twenty-eight days of residential treatment. The physician admits that prior to entering treatment on or about December 7, 2010, his alcohol usage had increased significantly in recent years. The Medical Director the treatment provider has opined that the physician has the diagnosis of alcohol dependence and that he is currently impaired in his ability to practice medicine and surgery in according with acceptable and prevailing standards of care due to the habitual and excessive use or abuse of substances that impair ability to practice.

    Action Taken

    In lieu of any formal proceedings, the physician has entered into a Step I Consent Agreement with the State Medical Board of Ohio in which the physician agrees with various terms, conditions and limitations including and not limited to the following.

    1. The physician’s certificate to practice medicine and surgery in the State of Ohio shall be Indefinitely Suspended.
    2. The physician shall be subject to Sobriety.
    3. The physician shall be subject to Drug and Alcohol Screens which shall be conducted at a Board-approved drug testing facility and collection site.
    4. The physician shall be subject to participation in a Rehabilitation Program such as AA, NA, CA or Caduceus.
    5. Conditions for reinstatement have been established.
    6. The physician shall be subject to Required Reporting including to all employers or entities with which he is under contract to provide health care services or is receiving training.

    In the event the physician violates any term or condition of the Consent Agreement, the Board reserves the right to institute formal disciplinary proceedings.
