The physician has entered into a Counter Settlement Agreement incorporating the original Settlement Agreement with the board subject to the following terms and conditions:
- The physician shall pay a Fine in the amount of $10,000.00 plus costs in the amount of $1908.36 to the board within thirty days.
- The physician shall complete five hours of continuing medical education in medical risk management within one year.
- The physician shall perform one hundred hours of community service within one year.
- The physician shall present a one hour lecture/seminar on performing a medically unnecessary procedure to the medical staff at a board approved medical facility within six months.
- The physician shall submit to a Quality Assurance Review of the physician's medical practice by a a licensed risk manager. The physician shall provide the Board's Probation Committee with the quality assurance report within sixty days and shall comply with all recommendations made by the risk manager.
