    Nature of Complaint

    In the physician’s license renewal application for the period of April 2009 – April 2011 the physician certified that he had completed or would complete the requisite hours of Continuing Medication Education (CME) for the CME period of January 2007 – January 2009. The Board required the physician to submit a log demonstrating that he had completed at least one hundred hours of CME for the specified period. The physician submitted documentation of 4.50 hours of Category I CME credits. The physician admits to the factual and legal allegations than he has not provided documentation that he has completed the required CME hours for the period of January 2007 – January 2009.

    Action Taken

    The physician entered into a Consent Agreement with the State Medical Board of Ohio in which he agrees with various terms, conditions and limitations including and not limited to the following.

    1. The physician is Reprimanded.
    2. The physician shall be subject to providing the Board with acceptable Documentation of CME.
    3. The physician shall pay a Fine in the amount of $5,000.00.
    4. The physician shall be subject to Required Reporting including to all employers or entities with which he is under contract to provide health care services or is receiving training.

    The reprimand shall not terminate. The physician shall not request termination of the terms of the Consent Agreement for a minimum of five years; nor shall he request modification to the terms, limitations and conditions for at least one year. In the event the physician violates any term or condition of the Agreement, the Board reserves the right to institute formal disciplinary proceedings..
