    Dr Sullivan does not listen to his patients, had not consulted my medical chart prior to my appointment, showed no familiarity with my case (this was my third visit with him in less then a month), and did not do a pain assessment on any of my visits even though I was there for persistent pain. I have no trust in his plan of care or competency.
