The physician failed to comply with the terms and conditions of the Stipulation and Final Agency Order dated 01-10-2008. The physician failed to successfully complete all portions of the CPEP Documentation Seminar within one year.
Action Taken-Second Stipulation & Final Agency Order
The Colorado Board of Medical Examiners has placed the physician on Probation for a period of five years subject to the following terms and conditions:
- The physician shall successfully complete the CPEP Documentation Seminar including the pre-program and the six month post-program within the required time period by CPEP.
- The physician's practice shall be monitored by a board approved monitor.
- The physician shall ensure that his practice monitor submits complete and timely quarterly reports to the board.
- The physician shall obey all federal and state laws.
Nature of Complaint:
The physician has been the subject of disciplinary action in another state.
The physician has been disciplined by the Colorado State Board of Medical Examiners for failure to adequately follow up on ordered laboratory examinations in a timely manner, failure to timely diagnose lung cancer, and failure to make essential entries in a patient's chart.

The physician has been issued a Public Reprimand by the Missouri State Board of Registration for the Healing Arts.
