Nature of Complaint:

The Physician prescribed a drug for other than medically accepted purposes and improperly managed medical records.

Action Taken:

The Board has conditioned and restricted the Physician's license for two years as follows:

1. The Physician is issued a Reprimand.
2. The Physician shall only practice in a pre-approved setting.
3. The Physician shall meet monthly with a supervising physician.
4. The Physician shall maintain a daily log of all controlled substances prescribed.
5. The Physician shall maintain a complete medical record including complaint, examination, diagnosis, treatment plan, response to treatment and prescription medications, including refills.
6. The Physician shall complete course in the following:
   1. Pharmacology.
   2. Medical records management.
   3. Professional boundaries training.
   4. Pain management.
7. The Physician shall submit to the Board a protocol for pain treatment including a contract.
8. The Physician shall meet quarterly with a designated Board member.
9. The Physician shall pay a $5000.00 civil penalty.

July 12, 2008-License Reinstated

The physician satisfactorily complied with the terms and conditions imposed upon his license.

The board has granted the physician an unconditional license to practice medicine in the State of Minnesota.
