Payment Policy:
Patients are responsible for all services rendered. Co-pay or non-insurance expenses are expected at time of service. $10 surcharge may be assessed if statements must be mailed for collection of co-pay. Bills over 45 days past due will be charged a late fee of 1% per month. Payment arrangements may be made in advance to avoid a late fee. 24 hour advance notification is requested to cancel an appointment. Returned check fee is $25.00.
