
Nature of Complaint:
The Physician failed to meet the standard of care in his care and treatment of a patient.
The Physician prescribed Vicodin on a continuous basis for complaints of pain. The Physician failed to perform comprehensive examinations, failed to document a discussion of the risks and benefits of treatment with narcotics, failed to document progress with treatment, and failed to monitor and document the effects of the medication.
The Physician has since completed further education in area of chronic pain management and made some changes to his practice, including more referrals to specialists.
Action Taken:
The Board has issued the Physician a Reprimand and ordered him to complete Education in the areas of prescribing practices and medical record keeping.
December 14, 2007-Order Terminated
The Physician has complied with the terms of his June 5, 2007 Order.
The Physician holds a full and unrestricted license to practice medicine in the State of Virginia.

The physician has been disciplined by the State of Virginia.
The physician failed to perform comprehensive examinations prior to prescribing Vicodin to a patient and failed to monitor the patient's progress and response to the course of treatment. The physician also failed to keep adequate medical records.
Action Taken:
The physician has agreed to Never activate his registration or to reapply for a license to practice medicine in the State of New York.
