The Physician has Voluntarily Surrendered the license to practice medicine in Connecticut.

Nature of Complaint:

The Physician has been the subject of disciplinary action in another state.

The Physician has been disciplined in the State of Connecticut for wirting prescriptions for a drug without adequate evidence of need.

The Physician also failed to update his practitioners profile in Florida to reflect the action taken by the State of Connecticut.

Action Taken:

The Board has placed obligations on the Physician's license to practice medicine in the State of Floridato include the following:

1. The Physician is issued a Letter of Concern.
2. The Physician is Fined $3000.00 plus costs.
3. The Physician shall complete Education in the areas of Medical Record Keeping, Prescribing Practices and the Laws and Rules course.

Nature of Complaint:

The Physician has been the subject of disciplinary action in another state.

The Physician has been disciplined in the State of Connecticut for writing a prescription for a drug without adequate evidence of need.

Action Taken:

The Physician has Surrendered the license to practice medicine in the State of New York.
