Nature of Complaint:

The physician has been the subject of disciplinary action in another state.

Action Taken:

The physician has Permanently Voluntarily Surrendered his license to practice medicine in the State of Pennsylvania.

Nature of Complaint:

The physician engaged in a sexual relationship with a patient while the physician was still treating the patient.

Action Taken:

The physician has entered into a Counter Settlement Agreement with the board incorporating the original Settlement Agreement subject to the following terms and conditions:

1. The physician is issued a Letter of Concern by the board.
2. The physician shall pay an administrative fine of $7,500.00 plus assessed costs of $1,940.22.
3. The physician shall obtain an evaluation from the Professional Resources Network and appear before the board if a PRN contract is required.
4. The physician shall perform one hundred hours of community service .
5. The physician shall successfully complete five hours of continuing medical education in the area of sexual boundaries.
6. The physician's practice is restricted in which the physician may not examine or treat female patients without the presence of a female Florida licensed health care provider for a period of five years. The physician shall appear before the board at the end of the practice restriction.

The physician has been disciplined by the State of Florida for having a sexual relationship with a female patient.

The board has accepted the Surrender of the physician's license to practice medicine in the State of New York.
