Disciplinary actions are actions taken to punish or restrict physicians who have demonstrated professional misconduct. HealthGrades reports state and federal disciplinary actions (if any) from the previous 5 years except when a physician's license has been revoked or surrendered.

Physicians with a disciplinary action in one state will often move to another state where they may have a clean record. Since HealthGrades painstakingly compiles disciplinary action information from all 50 states, you'll know if a physician has an action in more than one state.

Board Certification

Bridges to Excellence ® Medical Home
A medical home provides patients with access to quality health care from doctors who practice evidence-based guidelines; use appropriate health information technology; and demonstrate the use of "best practices" while being accountable for the quality and value of care. The BTE Medical Home program seeks to improve the quality of care for all patients by recognizing and rewarding physicians for demonstrating that they have adopted really good systems and processes of care, and are using those systems to deliver positive results in the management of their patients - in particular patients with chronic conditions. BTE Medical Home designation is achieved through a combination of BTE's Care Recognition and Physician Office Recognition programs.

BTE Medical Home recognition is a combination of BTE's existing programs:
  • Good Systems and Processes - means that the physician has achieved Level II or III recognition in the BTE Physician Office Recognition program, which demonstrates that the physician is collecting and using information to manage patient care, and the physician uses electronic systems to promote patient safety.
  • Good Outcomes - means that the physician has achieved Level 2 or 3 recognition in any two BTE condition-specific programs, which demonstrates that the physician is improving delivery of care and is measuring and reporting clinical performance.

The Cardiac Care Recognition (CCR) program seeks to improve the quality of care for cardiac patients by recognizing and rewarding physicians for demonstrating the delivery of high quality cardiac care. For Cardiac Care Recognition, physicians must meet standards associated with processes and outcomes in caring for patients with cardiovascular disease or who have had a stroke.

The performance measure areas, developed by American Heart Association/American Stroke Association and the performance assessment organizations, include:
  • Blood pressure control
  • Complete lipid profile
  • Cholesterol control
  • Use of aspirin or another antithrombotic
  • Smoking status and cessation advice and treatment

The performance measure areas, developed by American Diabetes Association (ADA) and the performance assessment organizations, include:

  • Eye exam
  • Smoking status and cessation advice or treatment
  • Complete lipid profile
  • Cholesterol control
  • Nephropathy assessment
  • Foot exam
