Nature of Complaint:

The Physician's surgical center was the subject of an onsite inspection. The inspection showed deficiencies in the following:

1. The crash cart did not have Heparin Inderal and Lanoxin. These medications were located in a locked cabinet 10 feet away.
2. Isuprel was on back order.
3. Surgery logs were deficient in that they did not list the type and duration of the surgery, the type of anesthesia used and post-operative care.

Action Taken:

The Physician has entered into a Consent Agreement with the Board to include the following:

1. The Physician has been issued a Letter of Concern.
2. The Physician is issued a Fine of $5000.00 plus costs.
3. The Physician shall complete 50 hours of community service.
4. The Physician shall complete the Laws and Rules Course.
