NATURE OF COMPLAINT

Violating a state law relating to the practice of medicine; unprofessional and unethical conduct; inability to practice medicine with reasonable skill and safety to patients by reason of illness

ACTION TAKEN

The physician’s is reprimanded and his license is suspended. The suspension is stayed upon his compliance with the following conditions and restrictions:

1. He shall obtain a primary treating physician to monitor and/or manage all medical or other care provided to him, and who shall make quarterly reports to the Board.
2. He shall obtain a treating neurologist who shall perform annual examinations of Dr. Urban and provide status reports to the Board.
3. He shall practice in a group setting.
4. He shall identify a work quality assessor at each location where he performs surgery, and who shall provide quarterly reports to the Board.
5. He shall meet quarterly with a Board member.
6. He shall be restricted in the manner in which he performs several surgical techniques.
7. He shall seek continuing educational opportunities regarding hands-off surgical techniques, indirect fracture reduction, and instrumentation that minimizes the risk of surgeon injury.
8. He shall notify the Board annually of his current clinical medical practice.
9. He shall review and comply with the institution’s policies and procedures for blood-borne pathogens.
10. He shall provide the Board with a list of all hospitals and skilled nursing facilities at which he currently has medical privileges.
