Nature of Complaint:

The Physician has been charged with failure to maintain adequate medical records in the case of a patient treated for an infection.

The Physician assumed the care of a dialysis patient while covering the patient's regular nephrologist. The Physician failed to prepare or submit a dictate note for the patient's chart. The Physician also charged Medicare for a consult instead of a Physical and History. He listed another physician as the referring doctor, although that physician did not see the patient until later.

Action Taken:

The Board has issued the Physician a Public Reprimand.
